Diabetes - Enhanced tailored treatment plans for type 2 diabetes

CPD hours:
3

Type 2 diabetes is a complex chronic disease that affects millions of Australians. Optimal management is required to prevent micro- and macro-vascular complications including cardiovascular disease, kidney disease, limb amputations, and blindness.

The value of a patient-centred, multidisciplinary approach to diabetes care has been reiterated in the literature. This may be implemented using tailored treatment plans, designed to meet the unique needs and goals of each patient.

Based on these plans, clinicians involved in the provision of care, may assist patients to identify their priorities in care, appropriately manage medications, improve treatment compliance, prevent complications associated with type 2 diabetes, and ensure the regular use of hypoglycaemic agents.

The following module offers learning through a case study. It highlights an example of a tailored treatment plan for type 2 diabetes and identifies patient specific factors, which may be used to guide individualised treatment.

Nurses play a significant role in working closely with patients to manage their diabetes. The module is aimed at anyone involved in the nursing care of a person with type 2 diabetes.

The rich text element allows you to create and format headings, paragraphs, blockquotes, images, and video all in one place instead of having to add and format them individually. Just double-click and easily create content.

Static and dynamic content editing

A rich text element can be used with static or dynamic content. For static content, just drop it into any page and begin editing. For dynamic content, add a rich text field to any collection and then connect a rich text element to that field in the settings panel. Voila!

How to customize formatting for each rich text

Headings, paragraphs, blockquotes, figures, images, and figure captions can all be styled after a class is added to the rich text element using the "When inside of" nested selector system.

At the end of this module participants will be able to:

  • Identify the value of the patient assessment and highlight the relevant information for type 2 diabetes management.
  • Understand the measures used to monitor glycaemic control and the benefits of individualised HbA1c targets.
  • Assess the risk of cardiovascular disease in patients with type 2 diabetes, and identify the recommended preventive measures based on risk.
  • Discuss the general goals of diabetes care and understand how they can be modified to meet the specific needs of any patient.
  • Review the use of hypoglycaemic agents in the management of type 2 diabetes and techniques for preventing hypoglycaemia.

At the end of this module participants will be able to:

  • This is learning objective 1.
  • This is learning objective 2.
  • This is learning objective 3.
  • This is learning objective 4.
  • This is learning objective 5.
  • This is learning objective 6.
